ICAM stands for Incident Cause Analysis Method. The ICAM analysis draws on the work of organisational psychologist and human error expert Professor James Reason to identify the underlying causes of the incident which are systemic in nature (eg. related to factors such as failures in design, procedures, training, auditing or risk management). James Reason did a lot of work on understanding ‘Human Error’ and the ICAM model is based on his Swiss Cheese, Defenses in Depth model of incident causation. This process ensures that the investigation is not restricted to the errors and violations made only by the people directly involved.

The ICAM provides a highly practical and structured method in which to conduct systematic investigations and is designed for OHS, operations and maintenance supervisory personnel who will either lead, facilitate or participate in both low and high level event investigations.

Choose from our range of courses below

ICAM Lead Investigator

This comprehensive course in best practice ICAM methods will equip participants with the qualifications to supervise incident investigations within their workplace and access the tools they need to manage their investigation team and the investigation itself. The course also covers how to reduce the risk of expensive litigation and criminal proceedings and establish the importance of correct methods of conducting an inquiry.

ICAM Team Investigator

This course teaches basic incident investigation skills. Successful participants will learn how to assist in investigations under the leadership of a trained ICAM facilitator. This course recreates an actual ICAM investigation, engaging participants in canvassing the scene, interviewing witnesses, and analyzing the evidence.

ICAM Governance

The ICAM Governance Workshop provides a high-level overview of the ICAM Process and discusses key organizational management perspectives of investigations. Approving Officers need to know what to review before approving investigation reports. Tools for quality assurance/checks are discussed to ensure that investigations and reports are completed to a high standard.

ICAM Master Class

This 2-day course provides an extensive breakdown in ICAM management and further develops the participants investigative skillset. Designed for lead investigators, this course will teach advanced practical skills and strategies to further enhance and compliment ICAM workplace investigations (including the research-based PEACE investigative interviewing method).

ICAM Train-the-trainer

This 2-day course is specially designed to equip participants with the qualifications and knowledge to train others in-house in either the Lead or Team Investigator courses. The course is also designed to help trainers improve their skills for training others. In this course, you will understand the various functions and techniques that you can use to maintain the interest of your students and to motivate them.

ICAM Refresher

This 1-day course is specially designed for personnel who have previously completed the ICAM lead investigators course and who have need to renew their 3 yearly accreditation.

Investigation Report Writing Workshop

This 1-day workshop is designed for personnel who are required to compile data and prepare in-house incident investigation reports or prepare presentations upon completion of incident investigations. The skills and knowledge possessed in order to conduct an incident investigation do not always go hand-in-hand with the skills and knowledge required to complete a well written Incident Investigation report.

5 Why's Workshop

This half day workshop is designed for personnel who are required to utilise the 5 Why’s problem solving technique as part of incident investigations to learn the key principles to ensure the process effectively identifies and addresses the root causes of an incident. In relation to incident investigations, the 5 Why’s technique is a well-known process that aims to distinguish an incident’s visible symptoms from the underlying latent root causes. Many organizations either use this technique for low level incidents or require this process to be completed in conjunction with the Incident Cause Analysis Method (ICAM).
